Uric Acid and Gout: The Midnight Big-Toe Test

Few pains are as memorable as a gout attack — often a big toe that becomes red, hot, swollen and so tender that a bedsheet feels unbearable, classically striking in the middle of the night. Behind it is a number many people have never checked: uric acid.

Where uric acid comes from

Uric acid is a waste product formed when your body breaks down purines — compounds found in the body's own cells and in certain foods. Normally kidneys flush it out. When you make too much or clear too little, it builds up in the blood and can crystallise in joints, triggering the searing inflammation of gout.

Not just the toe, and not just diet

Gout famously loves the big toe, but it can hit ankles, knees, fingers and wrists. And while rich food and alcohol get the blame, genetics and kidney function often matter more — plenty of careful eaters have high uric acid, and plenty of indulgent ones don't. It also travels with diabetes, high blood pressure and weight, so it rarely arrives alone.

The test

A simple blood sample measures serum uric acid; some doctors prefer a light fast. Broad reference ranges are roughly 3.4–7.0 mg/dL for men and 2.4–6.0 mg/dL for women, but the target during treatment is usually set lower to actually dissolve crystals. One important caveat: levels can be misleadingly normal during an acute attack, so testing is often repeated once the flare settles.

Foods worth knowing about

If your level is high, the usual suspects to moderate are:

  • Red and organ meats, and certain seafood (prawns, sardines)
  • Alcohol, especially beer
  • Sugary drinks and fructose-heavy foods

Helpful moves: plenty of water, weight control, coffee and dairy in normal amounts, and staying consistent rather than crash-dieting (which can trigger attacks).

Why testing beats guessing

Persistently high uric acid, even between attacks, can quietly damage joints and kidneys over time — so it's treated based on your numbers, not just how you feel.
